Key Responsibilities
- Engage and assess service users to understand their needs and level of functioning.
- Work with them to set goals and develop case plans to address concerns.
- Implement, review and modify case plans.
- Facilitate access to resources.
- Maintain and submit social reports.
- Reach out to individuals or targeted groups who may benefit from programmes.
